My Poor Patient

Today I went to my private office in Jamal Hospital. I was asked to see a patient by the director of the hospital. She was a middle aged obese lady with complains of low back pain. After a through evaluation, I found she had a mechanical back pain, so advised some pain killers and exercise. At the end she tokes out some money to pay my fee, but I instructed to paty to hospital cashier. While seeing another patient, I got the cashier with the patient in my office. Cashier frustrated that the patient is not paying her fee and patient ashamed of not having money. I got both frustration and shame. Frustrated on cashier why bringing a poor patient back just for money, and ashamed why I sent a poor patient to cashier. I asked to cashier to accept any amount she is able to pay. I was feeling a kind of guilt for accepting fee. Am I supposed to accept money from patient?
